Supports Coordination

MORC Support Coordinators play a key role, since they routinely assist the people MORC supports. Among the responsibilities of Support Coordinators:

Coordination of Treatment Plans:

  • Performs psychosocial assessments.
  • Provides or secures needed assessments.
  • Develops Individual Plans of Support with input from those who know the person the best, including family, caregivers, and friends.
  • Facilitates Person-Centered Planning meetings.
  • Conducts Training/In-service on implementation of individual plans.
  • Evaluates individual progress in the plan of support.
  • Coordinates the support services required by the plan of support.
  • Assists in resolving crisis situations.
  • Coordinates discharges.

Indirect Service Activities:

  • Liaison with schools, vocational providers, and others who are also assisting.
  • Authorizes needed services.
  • Does what is needed to enhance a spirit of gentleness in every aspect of the person's life.

Community Resource Management and Coordination:

  • Provides advocacy and linking with community agencies, schools, employers, etc.
  • Develops and mobilizes natural supports for individuals.
  • Provides guidance and/or assistance for individuals on accessing community resources and programs.
  • Assists individuals on safety and financial matters.
  • Maintains contacts with parents and/or guardians.
