Requirements

To be eligible for the MI Choice Waiver Program, persons must:

  • Be medically appropriate for a nursing home
  • Be Medicaid eligible
  • Be 18 years of age or older with a disability who would otherwise require nursing home care OR be over age 65 and in need of nursing home care.
  • Have a gross monthly income of $2,829 or less.
  • Personal assets must not exceed $2,000 a month
  • Reside in Macomb, Oakland, Livingston, Monroe, St. Clair or Washtenaw counties

All persons referred to MI Choice receive a telephone intake interview to determine eligibility for an in-home assessment. If eligible, a registered nurse and social worker visit the person’s home for a more in-depth assessment.

How to report suspected Medicaid fraud waste or abuse

Important Update (1-24-2022)

The Michigan Department of Health and Human Services (MDHHS) has asked that MORC Home Care strongly encourages all MI Choice participants to access their MIBridges account to review their contact information and update where necessary.

Any changes in phone number, email, or address should be reported to the Michigan Department of Health and Human Services (MDHHS). You can do this by going to the MIBridges website at www.michigan.gov/mibridges. If you do not have an account, you will need to create an account by selecting “Register”. Once in your account, when reporting changes, please make sure you do so in both the profile section and the Report Changes area.  The Report Changes area is what the local office will use to update the address for your case.
